Immunization Physicals are required for immigration to the United States.


People seeking to immigrate into the United States will be required to have received the following vaccinations:  hepatitis A and B, pertussis, tetanus and diphtheria, pertussis, polio, hepatitis A and B, measles, mumps, and rubella (MMR), meningococcal disease, varicella (chicken pox), pneumococcal disease, haemophilus influenzae type B (Hib), and seasonal influenza.

Important parts of an immigration physical.


Georgia Medical Clinic professionals will discuss your medical history with you and will look at or listen to your heart, lungs, eyes, abdomen, nose, lymph nodes, throat, as well as other parts of your body. Your immigration physical will also include blood testing and blood work that will screen for other conditions.

Dr. Rangaraj is a certified civil surgeon designated by the United States Citizenship and Immigration Services(USCIS). 
 
 
The examination includes the following:


  • Physical exam by our civil surgeon. Involves 2 visits.
  • Discussing and recommending vaccination requirements for the completion of your I-693 form.
  • Sealing the completed I-693 form in secure envelope and providing full copy records for patient




Examination fee is $350.00 (includes: Exam and filling out paperwork)

Payment due at time of service





Vaccination Requirements and blood work:


By law, for the immigration physical exam the required vaccines include:


  • Mumps, Measles, Rubella (MMR)
  • Tetanus and diphtheria toxoids
  • Hepatitis B
  • Varicella
    Influenza - only if in season (Sept 1st thru April 1st)

 

Blood work:


  • TB Gold test (if positive will require Xray and is separate charge)
  • Gonorrhea
  • Syphilis
  • Possibly MMR/varicella titers if no proof of vaccine is available 






You will be sent to outside facility for vaccinations and blood work and is a separate charge

 


 

When you are coming for your immigration exam, please remember to bring the following items with you.


    Please fill out the I693 form (including the name and A# on each page)


  • Passport or other gov issued photo ID (Must be current)
  • Medical Records if available (translated to English if necessary)
  • Proof of your vaccinations if available (Translated to English if necessary)
